Microneedling with exosomes: What to Do the First Week

Rancho Bernardo, San Diego, CA | Microneedling aftercare for real-life schedules
We hear it a lot: people feel an immediate glow after microneedling and then they want to do everything “right” so they don’t mess it up. The number one rule for microneedling with exosomes is simple: keep your skin clean, calm, and protected from San Diego sun for the first week. That means gentle cleansing, plenty of hydration, and no heat, sweat, or active products until your skin is ready.
First 24 to 48 hours after microneedling: keep it boring (and your results get better)
Right after microneedling with exosomes, your skin is doing a lot of behind-the-scenes work. We’re stimulating collagen and elastin production for skin repair, and you want to support that without adding irritation.
Do this today: Cleanse gently, hydrate, and treat your face like it’s in “quiet mode.” If you’re stepping outside in Rancho Bernardo or anywhere around San Diego, cover up and use broad-spectrum SPF once your skin can tolerate it.
For the first day
- Hands off. No picking, rubbing, or “checking” texture up close in the mirror.
- Skip makeup for at least 24 to 48 hours. This lowers the chance of irritation and clogged pores while your skin barrier is open and active.
- Avoid heat and hot water. No hot showers on the face, no steam, and don’t park your face over a boiling pasta pot.
- No sweat sessions. We generally recommend avoiding heavy exercise that causes sweating for 48 to 72 hours.
For day 2
Most people can do a gentle cleanse and moisturizer routine, morning and night. Keep it simple. If you’re going back into an office, put your hair up so it isn’t rubbing your cheeks, and avoid resting your face on your hands during meetings.
Days 3 to 7: the “looks a little weird, works really well” phase
This is usually when you might notice mild dryness, tightness, or a little flaking. That’s normal for many people. The biggest mistake we see is trying to “fix” it with scrubs or strong products. Don’t. Let your skin do its work.
Sun exposure is the other big one. San Diego sun feels gentle until it isn’t, especially when your skin is fresh after microneedling. If you’re out running errands around Rancho Bernardo, Poway, or 4S Ranch, use a wide-brim hat and avoid midday outdoor time this week if you can.
Keep avoiding: retinol, exfoliating acids, abrasive scrubs, and strong active serums for about a week. For some skin types, we’ll hold those 7 to 14 days.
- Don’t pick peeling skin. Picking can lead to irritation and uneven pigmentation.
- Hold off on swimming. Pools, ocean water, and hot tubs increase risk of irritation for at least 48 to 72 hours, often longer if your skin is still sensitive or compromised.
- Skip saunas and steam rooms. Heat and bacteria are not your friends this week.
The simple product trio we like after microneedling: cleanse, hydrate, protect
You don’t need a complicated routine after microneedling with exosomes. You need the right categories, used consistently.
1) Gentle cleanser
No fragrance-heavy foaming cleansers, no scrubs, no cleansing brushes. Use lukewarm water and pat dry.
2) Hydration and barrier support
Look for hyaluronic acid for hydration, and barrier-repair ingredients like ceramides and peptides. This is where your skin tends to feel the most comfortable.
3) Broad-spectrum SPF (often mineral is easiest)
This is non-negotiable in San Diego. Apply daily once your skin can tolerate it, and reapply if you’re outside. Hats help too.
If you’re not sure when to bring back your actives, let us know what you use and we’ll give you clear timing. We’d rather you ask than guess.
When to reach out to us (and when it’s just normal healing)
A little redness, warmth, and mild flaking can be part of the first-week picture. Many of our clients tell us they feel comfortable during microneedling, and they love how refreshed and clear their skin looks as the days go on.
